He clearly had an unfinished business. And we saw his ghost so he didn't move on until the end of the episode
Also, why did the ghost appear until now? I mean, he died like 50 years ago. Why did his ghost appeat now?
And did Hook lie about never being on Pleasure Island? Because the coin went there in the map when they used the spell. So, if the coin said he died there, then that means Hook was there too.
I took it that the ghost was a figment of David's imagination, brought on by a lack of sleep and his new obsession over his dad thanks to the coin.
As for the other two points; plotholes, plotholes everywhere

Robert could have been anywhere. Logistics of the UW are unknown but if it really collects the souls of all the realms of storythen it's bigger than it looks. Robert probably just missed David and had to wait until the phones were reinstalled. Plus, plot.
David's father had died in a ravine, which was always the case. They were on mainland EF. Why did the coin stop there? Guess it depends on how the magic worked (which does whatever the plot demands). David likely wanted to know where his father went after he left, or maybe David and Hook didn't turn enough. XD
Who said he wasn't? It's not like Snow and David went through the whole giant ass Underworld cemetary to find out the status of every dead person they knew. And just from what we saw, there were a ton of people down there, so the chances of just running into him there would be slim.
But didn't he come back there after 5x21? I assume the rules are back to normal
But Hades wasn't killed. He was destroyed, by the Olympian crystal, which means no Underworld. He doesn't exist anymore. Not as a soul, not as anything other than an inanimate pile of ash.
Oh true! Well, then the rules are definitely changing now that Arthur is in charge.
I just rewatch the episode. Crazy theory here. What if August has ulterior motive for showing Hook the storybook pictures? This does not take away that Hook killed Robert. But what if he doesn't like the fact Hook is with Emma? He may like Emma himself, being with her all that time and want this fact to destroy their relationship. Or he is just being protective of Emma and he wants Hook to prove himself even more by telling her the truth of what happened? His reaction to hearing that Emma was going out with Hook was of surprise. The episode featured Pinocchio lying and his nose grown. Sure it may a homage to the animation, but what if it parallel August lying now. Well, not really lying, just have an ulterior motive.
The pages don't really show that Hook killed Robert. It simply sparked Hook's memory that he did kill Robert because he recognized him.
True, the pictures do not show Hook killed Robert, but it doesn't take away my theory of August's possible ulterior motives. August is well verse on what happebed in all stories whether he was there or not for the actual event. For example, he knew how Baelfire went to the Land Without Magic without Rumple. So I bet there is a story somewhere in the book that depict Hook killing Robert. So August know of Hook's past and put two and two together for his ulterior motive.
